Summary: The purpose of this survey was to investigate the barriers to the provision of employer-sponsored health insurance coverage and to describe the premiums and other characteristics of health plans offered by employers. With the goal of remedying the previous lack of state-level data, the survey was conducted to aid in defining problems in the employment-based insurance market and in analyzing the impacts of states' policy options.
